01 / THE PARTIES
Alafia Swampers (“Band”) and the client named above (“Client”) agree to the performance described below, subject to the completed terms and both parties’ signatures.
02 / THE SHOW
03 / THE MONEY
Any extra performance time or additional expense requires mutual written approval. Deposit refund conditions and cancellation charges must be expressly completed below.
04 / STAGE & SOUND
Client will provide safe access and the agreed performance area, power, and venue arrangements. Band will provide the agreed performers and equipment. Unsafe conditions must be addressed before performance proceeds.
05 / CHANGES & CANCELLATION
Date, location, fee, or performance changes require agreement in writing by both parties. A substitute act requires Client’s written approval. Complete these terms before signing.
06 / THE AGREEMENT
This completed document and any identified attachments state the parties’ agreement for this performance. Amendments require both parties’ written agreement. Each signer confirms authority to act for the party named. Both parties should retain the same completed, signed copy.
